Ambulance Ramping is putting the Safety of your Team at Risk: Why Staff First Aid Training is a must

Have you noticed? Ambulance ramping is at crisis point and the dire consequences are back in the media. We have seen the system continue to perform poorly to be able to support our community. Recently a single mother from Qld, passing away after waiting for an ambulance for over 1.5 hours, that never came. We continue to see media coverage reporting that Ambulance ramping is worst in South Australia, with Queensland following very close behind. All states are experiencing an all-time high ambulance ramping crisis.

According to the AMA Ambulance Ramping 2023 report:

Australia is experiencing unforeseen levels of ramping, with reports of people needing to be driven to the emergency department as there are no available ambulances, and people dying waiting for an ambulance.

I would like to take a moment here to praise our paramedics. This is not a fault of our workers, it is the system. We’ve heard desperate stories in the media, that paramedics are so personally concerned about the system, that some have used their own vehicles to transport patients to hospital. That certainly is dire, and clearly shows the dedication and passion of these amazing people.

This is all shocking, however is the current reality.

You might be wondering, how does that affect you? How does it affect your business?

The cold hard truth is this has ENORMOUS impact to you and your business!

In the past, if a critical incident occurred at your workplace, you could call ‘000’ and reliably know that help was not far away.

In November 2023 – is that the case? Can you count on that?

Workplace safety needs to be a priority even more so now, with the stress on our emergency services. A few workplace statistics to put things into perspective for you.

  • 25,000 out of hospital Cardiac Arrests happen every year in Australia, chance of survival is 12% of less if there is a delay in providing life saving measures such as CPR. (Medical Journal Australia)
  • 560,000 Workplace injuries occur every year and 52% result in workers compensation claims (Australian Institute of Health and Welfare)
  • Code 1 (life threatening) calls can be waiting 1 -2 hours for an ambulance to arrive in Brisbane (ABC)
  • A casualty requiring CPR will be brain dead within 4-6 minutes if CPR is not performed.
  • A casualty with an arterial bleed will bleed out in 3-4 minutes.
  • A casualty experiencing an anaphylaxis emergency needs immediate first aid response.
  • Choking and drowning are both silent first aid emergencies and first aid must be administered immediately. There is no time to wait for the ‘first aid person’ on duty to arrive.

Here is a snippet of what has happened in the past – Is it continuing?

According to News.com.au – “Police attending calls of people threatening suicide has jumped 58 percent in six years” and “almost 1000 calls per week from people threatening suicide”. When Ambulance services are overwhelmed, Police services are stepping up where they can help.

Combine both the physical and mental health impacts to our community during and post pandemic and this is causing enormous strain on our medical services and contributing to the ramping issues we are witnessing.

Australia’s health system is continuing to fail patients suffering from poor mental health, with more patients needing acute care and facing long waits in emergency departments.

The ramping crisis is continuing as the hospitals simply do not have the in-patient capacity for the current needs of the community.

Until there is national overhaul to the system, we need to work together to find ways to manage through this crisis.

Ensuring Safety and Preparedness with First Aid Compliance in School

Education and learning new skills are incredibly important. We only need to ask our educators that!

As parents and carers, we send our children off to day care, kindy, prep, primary or high school to learn skills that will hold them in good stead for a lifetime. We trust that our educators have the skills, confidence and professionalism to support our children to navigate the curriculum. First aid compliance in schools is highly important.

What about the first aid compliance requirements to be prepared in schools?

  • Who educates the educators?
  • How do they stay current with their skills?
  • What do they need to know?

Who educates the educators? Teachers are often in the position as first responders.

Do you recall the tragic incident in 2017? – whereby a young 5-year-old student with special needs (who often mouthed foods and objects) choked on a nectarine stone and died whilst at school. An inquest was held in 2022, to prompt questions about the requirements for first aid compliance in schools and whether the teachers were able to effectively respond.

Every workplace in Australia, including the education sector are required to:

Let’s explore this in more detail to review the requirements for first aid compliance in schools and education settings and what that exactly means for educators.

  • Provide First Aid equipment
  • Ensure each worker at the workplace has access to the equipment
  • Provide access to facilities for administering first aid
  • Provide an adequate number of workers that are trained to administer first aid, or workers to have access to an adequate number of other people who have been trained to administer first aid.

Access to equipment:

Think first about the vast variety of situations in an education setting where equipment may need to be on hand, to respond effectively. (this list is not exhaustive!)

  • Swimming carnivals
  • Classrooms such as Industrial Skills, Science Labs, Hospitality
  • Playgrounds
  • Camps/Excursions
  • Sporting carnivals
  • Sports Days/Inter School Sports
  • Special Needs Classrooms
  • School Pick Up/Drop off

Do you have access to First Aid Kits (stocked with the right equipment?)

What does the Code of Practice say?

The Code of Practice also requires workplaces to determine ‘an adequate number of workers’ that are trained to administer first aid.

The following ratio’s are recommended:

  • Low Risk Workplaces – one first aider for every 50 workers
  • High Risk Workplaces – one first aider for every 25 workers.
  • Remote High-Risk Workplaces – one first aider for every 10 workers

Let’s crunch some numbers (these figures taken from the 2019 Annual Report for Ipswich State High School, Qld)

  • 1763 students
  • 131 teachers
  • 76 non-teaching staff

Perhaps you are thinking 207 teachers and non-teaching staff (workers) in this example. If the school/education setting was ‘low risk’ – that equates to approx. 4 First Aid Officers required.

What do you think?

Let’s go back to the Code of Practice to delve further.

“When considering the size of your workforce, you should include contractors, subcontractors and volunteers you engage. This may mean the size of your workforce may vary over time. For the purposes of deciding who requires access to first aid, you should consider the maximum number of workers you may engage at one time. Generally, a larger workforce requires more first aid resources”

We can see that we also need to include other ‘workers’ into our consideration of factoring first aid requirements.

Is there more to include? … You should also consider:

  • the particular needs of workers who have a disability or a known health concern for example asthma or allergies, and
  • others at your workplace who are not your workers, for example:

Let’s go back to our original numbers.

The figure we need to realistically consider in establishing the appropriate numbers of first aid officers is:

1970 (as you need to factor the number of people which includes students too, in the ‘work’ area).

This is the number of people we include in your First Aid Risk Assessment. We now move to the next stage which is an understanding of the risks within your school/educational setting:

Does your school/educational setting have:

  • Children or teachers diagnosed with anaphylaxis?
  • Children or teachers diagnosed with asthma?
  • Children identified with special needs and/or on regular medication?
  • Students who play contact sport?
  • Teachers that are managing high risk behaviours?

Now think of the environment:

  • Does the setting have an on-site pool?
  • Located on a main road?
  • Multi storey building with difficulty accessing site/lots of stairs?
  • Provide cooking/hospitality classes, industrial skills classes, science labs?
  • Is it in a remote regional/rural setting?
  • What is the distance to medical help, distance to a hospital?
  • What is the expected response times for arrival of emergency services?

Remember to add:

  • What kind of incidents have happened in the last 12 months?

Conduct a First Aid Risk Assessment to understand how many First Aid Officers you require

Essentially you are determining a first aid risk assessment to ascertain:

  • What First Aid equipment you need (including epi-pens, Ventolin, spacers, AED’s)
  • How many First Aid Officers you require to be able to effectively respond.
  • Where you require First Aid Equipment and facilities (portable, fixed, available in vehicles etc)

Once you have conducted a First Aid Risk Assessment to include all of the above, then the compliance for your schools will be better understood.

There may be overarching requirements specific to your school/educational setting documented further in school or centre policies and these should be referred to for guidance and included in your Risk Assessment.

Often education settings have very specific requirements for both the training and equipment requirements for effective response of anaphylaxis and asthma.

Every Business needs to know:  Managing Workplace Psychosocial Hazards and Risks

Why is there all this talk on LinkedIn and other platforms about the impending changes to the WHS regulations for managing the risk of psychosocial hazards?

What is all the chatter around Psychosocial Risk?

Hasn’t there always been an obligation placed on businesses and organisations to manage both health and safety risk anyway?

Why is there a need for change?

The WHS regulations have changed to provide greater clarity for businesses to manage psychosocial risks

It is simple really.

This shouldn’t be a new concept for any business; however businesses just haven’t been able to necessarily approach psychological health and safety with effective ways to prevent or reduce psychosocial risk in the workplace. It turns out this has been quite difficult to achieve and the guidance not necessarily simple to follow.

If you reflect over the past decade, we can see by looking at the stats released from Workcover Qld that physical injuries are trending down, however psychological issues are trending up. Reports such as the Marie Boland review, helped shine a spotlight on the gaps for effective management and implementation of measures.

With this change in trend, are businesses effectively managing these risks?

The changes in regulation isn’t meant to make life more difficult for businesses, instead it is to step out the ways in which businesses can make a proactive and positive step to improving the psychological health of their workers.

Mental Health Statistics

Healthy, happy workers:-

  • are productive workers.
  • have improved relationships with family members and friends.
  • have less absenteeism.
  • are less likely to leave the business

The change in regulation shall put the spotlight on psychological safety

The proposed change, shifts information on how to prevent harm to psychological health from being:-

  • guidance (you should do it this way),
  • to Regulation…..you MUST do it this way.

In Qld, a Code of Practice is being implemented on the 1st April, 2023. This will act as the go-to guide to explain how a business can meet its WHS obligations.

I wouldn’t let the word ‘MUST’ distract businesses from the essential goal of why the regulation is changing. The important part of the messaging is that this is designed to assist and is a time to embrace the help!

Let’s talk about psychosocial hazards. They are anything at work that may cause psychological harm.

These hazards can come from or relate to:-

  • the way work is designed and managed,
  • the working environment
  • plant at a workplace
  • workplace behaviours or interactions (e.g. bullying, harassment, discrimination, aggression and violence)

Psychosocial hazards may exist in your workplace and be unavoidable. (e.g. working in a health care settings whereby you encounter violent, aggressive or abusive patients).

What you need to know is what are the things you should reasonably know about the risks, what can you do to reduce the risk? Or preferably eliminate the risk altogether?

A psychosocial risk is a risk to the health or safety of a worker or other person from a psychosocial hazard.

If these risks are not well managed, it would be reasonable to think that a worker may experience a psychological injury impacting workplace safety.

Let’s discuss hazards here, so you can see what I am referring to.

Bullying (Workplace Behaviours)

I had a friend who worked in an office environment, whereby a team leader would poke fun at my friend about her spiritual beliefs. The co-worker often referred to her as a ‘witch’ amongst peers. My friend felt uncomfortable with this kind of banter, and did address this directly with her work colleague, however this continued relentlessly, and other work colleagues began to join in on the ‘sledging’ and in their minds, it simply became an office joke, which continued and co-workers regularly taunted her as being a ‘witch’ and ‘riding on her broom’.

Understandably it all became too much for my friend, who left the company informing the CEO of the ‘bullying’ culture that existed within the company.

How Mentally Healthy is your workplace really?

Clearly it was not a mentally healthy workplace and bullying behaviours went on unchecked. In this instance, my friend took matters into her own hands, leaving the company as recognised, remaining in this toxic culture, would not be good for her mental health.

It was repeated, unreasonable behaviour and was a humiliating experience for her.

While we often think about bullying as an individual or interpersonal issue, research commissioned by Beyond Blue shows that broader environmental factors – such as poor organisational culture and a lack of leadership – are in fact the main drivers.

Low Job demands (Work Design)

Let’s say that you have a business that has work such as a production line in a food manufacturing company. The role of your worker may be to provide placement of a food item on the production assembly line. The design of that role may have impact to both physical and mental health, so you need to look at the risks and review the work design.

How would you change it?

Measures may include, rotation of tasks, cross training in other areas of production, anti-fatigue mats provided, opportunity for workers to partake in other roles such as a Health and Safety rep, First Aid Officer, allowing suitable break/rest periods, allowing workers to chat whilst working on the production line etc.

These are some of the ‘measures’ to reduce both physical and psychosocial risks. In this instance, the measures help to reduce the risk, however, does not eliminate the risks altogether.

It isn’t overly hard to determine the risks, simply conduct a risk assessment and work out ways that you can better manage. Ask your workers! They often have the best solutions. There are wonderful risk assessment tools available with Worksafe Qld and People at Work.

High Job Demand (Work Design)

Another example of a psychosocial hazard, which may impact on a person’s psychological health is high job demand. I worked in the finance industry when I was younger and the company was about to embark on significant transformational change. It was enormous impact to both clients and workers with several branches closing its doors. Two young women led the transformation project.

Frequently they worked very long days, some days finishing work at 1am, only to be back in the office by 7 or 8am the next day. The team on the project were also expected to work long days. I was in the latter stages of my first pregnancy and working until 9pm or 10pm on some occasions.

The issue with this was not the fact that it may be a one-off requirement for the job or demands of the project. There will always be moments whereby there are extra requirements or work demands. Especially in today’s modern workplace. It is about sustaining this over a long period of time and my concern was, that although the two young leaders achieved great success for the business, it presented significant risk for them personally and also set a precedence.

Imagine what this means for the next person who enters a similar role, to achieve the same tasks, within the defines of an 8-hour day? It is nigh impossible.

It is important for businesses to understand both the physical and psychosocial risks that impact workers

Did the business know that this was occurring? Yes it did. Were the risks for both physical and psychosocial risk identified? No it wasn’t – there did not appear to be measures in place to manage fatigue, burnout, stress, working in isolation (e.g. 1am finish times and working alone in an office).

What kind of physical affect did this also have on them?

The daily long hours of work became an expectation to meet the project timelines for an unreasonable period of time. The psychosocial hazards were evident, yet the risks were not well managed.

The focus should be on preventing harm

The changes in the regulation essentially means that ‘officers’ of a business (PCBU), are not able to turn a blind eye to what is happening within a business. We see evidence that psychological injury claims are on the rise, so the focus should be on:-

  • Preventing harm
  • Intervening early
  • Supporting recovery
  • Promoting health

The key message here is to get started.

Self-care strategy books

Australian workers suffered a higher level of work burnout than other countries surveyed. (sourced from the latest Microsoft Work Trend Index). The trend index report, published on Sept. 22, showed that 62% of Australian workers reported being burned out at work, compared to the global average of 48% of employees.

It’s time to make change. There are a number of great tools available to support you. Businesses shall be required to undertake a risk assessment approach. Begin with a free and validated Australian Psychosocial Risk Assessment Survey.
